    Make a meaningful impact by helping Louisiana residents move from nursing facilities into supportive community settings. As a My Choice Transition Coordinator (TC), you will lead and guide transition teams across regional offices, support coordination agencies, Medicaid Managed Care Organizations (MMCOs), and Medicaid providers to ensure smooth, person-centered transitions. This role offers the opportunity to solve complex challenges, provide technical assistance, identify barriers, and help shape policies that strengthen community capacity.     Minimum Qualifications

    Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

    EXPERIENCE SUBSTITUTION:

    Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

    Job Duties and Other Information

    Job Duties:

    + Plans, implements, and monitors institutional-to-community transitions for individuals across assigned regions. Conducts transition assessments, determines eligibility for OAAS HCBS services, and develops individualized transition plans using information gathered from interviews, records, and observations. Coordinates housing searches and related paperwork, ensures access to needed behavioral health and support services, and facilitates communication among transition team members. Provides technical assistance on complex cases, conducts pre- and post-transition visits, and delivers intensive case management to support successful community living. Evaluates transition outcomes, addresses post-transition issues, and offers training and support to providers, nursing facility staff, and other stakeholders.

    + Develops and maintains strong working relationships with local Long-Term Care Ombudsman staff to support successful transitions. Provides ongoing communication, guidance, and training to Regional Ombudsman teams, ensuring effective collaboration and coordinated assistance for individuals transitioning to community-based living.

    + Maintains and updates an inventory of community resources, services, and products from vendors, local businesses, churches, and charitable organizations. Builds and nurtures relationships with these groups to ensure timely access to needed services and products at the most cost-effective rates.

    + Identifies and communicates necessary updates to policies related to nursing facility transitions. Collaborates with other program offices and Medicaid as needed, responds to emergency activations, and performs additional duties or assignments in accordance with LDH policy.

     

    The ideal candidate should possess: Position requires knowledge of long-term care service delivery to older adults and/or people with disabilities. Experience with one or more of the following is preferred: transition planning from nursing facilities; transition planning from ICDFs/DD; service delivery to persons with mental illness; experience with support coordination/case management; social services coordination; Medicaid or program office functions on the state or regional level; HCBS enrolled provider agency management.
