About the Role
As a Physical Therapist at our facility, you will evaluate, plan, and deliver evidence-based physical therapy services to residents in accordance with physician orders, state practice acts, and federal regulations. You’ll have the opportunity to make a powerful impact on each resident’s functional abilities, quality of life, and overall well-being.
What You’ll Do
+ Conduct comprehensive evaluations and develop individualized treatment plans
+ Provide therapeutic interventions to improve strength, mobility, balance, and functional independence
+ Document assessments, progress notes, and therapy minutes in compliance with facility standards and Medicare/Medicaid guidelines
+ Coordinate care with an interdisciplinary team of nurses, therapists, physicians, and social services
+ Participate in care plan meetings to support effective discharge planning
+ Educate residents and families on exercises, safety, and strategies for continued success
+ Ensure therapy equipment is used safely, maintained, and properly sanitized
+ Assist with monitoring department performance indicators and maintaining regulatory readiness
+ Promote a positive, compassionate, resident-centered care environment
Required Qualifications
+ Active, unrestricted Physical Therapist license in the state
+ Graduate of an accredited Physical Therapy program
+ Current CPR certification (or willingness to obtain)
+ Valid driver’s license in the state of employment
+ Knowledge of skilled nursing facility operations and long-term care regulations preferred
+ Strong communication, documentation, and clinical reasoning skills

Physical Requirements
+ Ability to lift, push, pull, and assist residents as needed
