    Aspirus Health in Duluth, MN is seeking a COMMUNITY HEALTH SPECIALIST to join our COMMUNITY HEALTH team!

     

    The Community Health Specialist is responsible for managing and coordinating Community Benefit activities across the Aspirus system and for developing innovative ways to advance Aspirus’ efforts at population health management.This position is actively involved in the community as Aspirus’ community health liaison and will facilitate community health improvement partnerships among hospital, public health, and community partners.

     

    This position is responsible for Aspirus system compliance with Section H of IRS Form 990 and 501r and federal reporting regulations, ensuring completion of Community Health Needs Assessment (CHNA) reports and implementation strategies across the system.

     

    This position will have system responsibilities for ensuring compliance and accountability with all Community Benefit and CHNA goals, strategies, and expectations.

     

    This position will work with the Community Health Improvement team, which will regularly discuss activities and changes, and identify opportunities for intra-system collaboration.

     

    HOURS: Full-Time or 1.0 FTE, 80 hours every 2 weeks.

     

    WAGE: $24.84 - $37.87

    Experience/Qualifications

    + Knowledge of how hospitals and public health partners can effectively improve community health by working together to meet community benefit and community health improvement requirements. Bachelor’s degree in community health education/public health, public administration or policy, social work, or related field required. Master’s degree preferred.

    + A minimum of three years in planning, program development and administration of health or social services sector working with traditionally underserved communities.

    + Knowledge of the principles, practices, and regulatory environment of community health needs; program development, implementation, and evaluation; and population health.

    + Experience working with healthcare systems and community-based organizations.

    + Experience or knowledge of grant writing processes and procedures.

    + Must have a pro-active approach to Community Benefit: anticipating issues and developing new procedures on existing approaches, systems, or processes.

    + Requires a dynamic, energetic, highly motivated, and results-oriented individual with the ability to balance a multitude of organizational priorities.

    + Demonstrates political savvy in dealing with corporate, executive and physician constituents.

    + Working knowledge of budgeting and spending tracking & monitoring.

    + Demonstrated effectiveness in interpersonal and communication skills as well as an ability to work in teams and facilitate group consensus across a multitude of stakeholder groups.

    + Communication skills will be very important – motivational, not afraid to be speak up and be a voice for the cause.

    + Ability to use organizational tools and prioritize work.

    + Passionate about people and the health of the community.

    + Ability to see the big picture as well as the small.

    + Collaborative; must partner well with others both internally and externally.

    + Strong sense of accountability and excellent work ethic, goal oriented.

    + Willingness to be a change agent and positive role model.

    + A commitment to serving vulnerable populations.

    + Knowledge of health care reform and Affordable Care Act requirements.

    + Possesses personal initiative and sound judgment. Must be a strategic thinker, well-versed in research techniques.

    + Is highly organized and detail oriented.

    + Possesses excellent language skills, oral and written.

    + Possesses above-average knowledge of technical infrastructure including computer software and equipment.

    + Possesses excellent interpersonal skills.
