ASPIRUS ST. LUKE'S BEHAVIORAL HEALTH CLINIC in Duluth, MN is seeking a PHYSICAL THERAPIST-HC to join our SKILLED HOMECARE team!
Salary Range: $39.92 - $57.89 Hourly
Schedule/Hours: Weekdays, DAY ONLY, 8 hours
Weekend Requirement: Generally No Weekends
FTE: FULL TIME, 1.000000 FTE
The Physical Therapist is responsible for providing physical therapy to Home Care/Hospice patients and primary case management when indicated. Responsible for timely and accurate communication and documentation relative to physical therapy delivery. Provides clinical supervision for the Physical Therapist Assistant. Promotes and develops physical therapy services in Home Care/Hospice.
MINIMUM QUALIFICATIONS
Education: See Licensure/Certification/Registration requirements.
Experience: N/A
Licensure/Certification/Registration: Current Minnesota Physical Therapy license. Wisconsin Physical Therapy license within six (6) months of hire. Wisconsin Background Study upon hire and renewed per Wisconsin state law.

NOWLEDGE, SKILLS AND ABILITIES
General knowledge of the principles of receiving, storing and issuing supplies. Principles of packaging, shipping and mail handling. Ability to read and perform mathematic calculations at the high school graduate level; to exchange oral and written communication; to organize and prioritize workload; to work independently or as a team member; to establish good working relationships with co-workers, medical and Hospital staff and exhibit good interpersonal communication skills.
+ READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
+ WRITING - Basic: Ability to write simple correspondence.
+ SPEAKING - Basic: Ability to speak simple sentences.
+ MATHEMATICAL SKILLS - Basic Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
+ REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
