Skilled Nursing Facility / Admissions Coordinator – Woods Cross, UT

Pay: $19 - $20/hour

Schedule: Weekly Rotating shifts, Monday–Friday (8:30 AM–5:00 PM or 9:30 AM–6:00 PM)

Position Overview:

The Admissions Coordinator plays a vital role in managing the admission process for patients entering the skilled nursing facility. This position involves coordinating referrals, verifying insurance, and ensuring a seamless transition for new residents.

Key Responsibilities:

+ Review clinical documentation to determine level of care needed.

+ Verifies physician orders.

+ Obtain pre-admission documentation from referral sources to determine if agency can care for the patient adequately.

+ Communicate with patients, families, and healthcare providers to gather necessary documentation.

+ Maintain accurate records in electronic medical systems.

+ Coordinate scheduling and follow-up for new admissions.

Qualifications:

+ Previous experience in healthcare admissions, intake, or patient services preferred.

+ Strong organizational and communication skills.

+ Ability to multitask in a fast-paced environment.

+ Proficiency with EMR systems and Microsoft Office Suite.

This is a Contract to Hire position based out of Woods Cross, UT.

Pay and Benefits

The pay range for this position is $19.00 - $19.00/hr.
