Long-Term Acute Care(LTAC) Registered Nurse (RN)

The LTAC RN is a skilled and compassionate nursingprofessional responsible for delivering comprehensive care to patients withcomplex, long-term medical conditions. This role requires strong clinicaljudgment, adaptability, and the ability to manage multiple priorities in afast-paced, high-acuity environment. The RN collaborates with physicians,interdisciplinary teams, and support staff to ensure safe, effective, andpatient-centered care.

Essential Duties and Responsibilities

• Apply the nursing process to assess, plan,implement, and evaluate care for patients with chronic, critical, and medicallycomplex conditions

• Conduct thorough patient assessments, includingvital signs, physical exams, health histories, and monitoring of invasive linesand devices

• Manage ventilator-dependent patients and provideadvanced respiratory care

• Administer medications, treatments, and IVtherapies as prescribed

• Monitor and document patient responses tointerventions and changes in condition

• Collaborate with healthcare providers to developand adjust individualized care plans

• Provide wound care, pain management, andmobility support for patients with extended recovery needs

• Educate patients and families on diseaseprocesses, medications, discharge instructions, and self-care

• Maintain accurate and timely documentation inthe electronic health record (EHR)

• Adhere to infection control, safety, and qualitystandards

• Respond promptly to emergencies such as rapiddeterioration, sepsis, or code situations

• Support patient advocacy and uphold patientrights and confidentiality

Minimum Requirements

• Current Registered Nurse License in the state ofpractice

• Minimum one year of professional nursingexperience; LTAC, ICU, or Med-Surg experience preferred

• Completion of Amergis' Competency Program (ifapplicable)

• Current BLS certification (ACLS stronglypreferred)

• Proficiency in managing ventilators,tracheostomies, and complex wound care

• TB screening and health clearance (as requiredby contract or regulation)

• Compliance with all federal, state, and localregulations

• Successful completion of new hire orientationand LTAC-specific training

• Understanding of patient confidentiality andHIPAA regulations

• Strong communication skills and proficiency inEnglish

• Basic computer literacy

• Must be at least 18 years of age
