Qualifications
+ Citizen of the United States
+ Strong critical thinking, communication, and care coordination skills.
+ Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
+ Two (2) years or more experience in critical, ambulatory or primary care.
Licenses & Certifications
+ A current license in at least one (1) state, territory, or commonwealth in the U.S. or the District of Columbia
+ Current Basic Life Support (BLS) certification from American Heart Association
+ Current ACLS certification through American Heart Association
Duties
The RNs and LVNs assigned to the Patient Aligned Care Team (PACT) in the outpatient clinic play a vital role in the delivery of comprehensive and coordinated primary care to Veterans. As core members of the interdisciplinary PACT team – which includes a Primary Care Provider (PCP), Licensed Vocational Nurse (LPN), and Clinical Support Staff – the RNs and LVNs are responsible for the case management, health promotion, disease prevention, and chronic disease management of assigned patients.
Summary:
The Ambulatory RN provides comprehensive, evidence-based nursing care to Veterans in an outpatient setting. This role supports primary care services within a Patient Aligned Care Team (PACT) model, focusing on health promotion, disease prevention, chronic disease management, and care coordination.
Key Responsibilities:
+ Deliver direct patient care and coordinate services across the continuum of care.
+ Perform patient assessments, triage, health screenings, and documentation.
+ Provide education and coaching on wellness, self-management, and disease prevention.
+ Administer medications, immunizations, and perform point-of-care testing.
+ Collaborate with interdisciplinary team members to ensure holistic, patient-centered care.
+ Utilize clinical protocols to order labs, tests, and enter consults as appropriate.
+ Maintain competency in BLS/ACLS and adhere to VA standards and policies.
