    Job Summary

    The Fertility Preservation Program at Michigan Medicine provides comprehensive, integrated, multidisciplinary care for pediatric and adult patients who wish to preserve fertility before potentially gonadotoxic treatment, as well as those who need fertility care after completion of treatment.

     

    The Fertility Preservation Patient Navigator is a knowledgeable, compassionate healthcare professional who will guide patients and families through the medical, emotional, and logistical aspects of fertility preservation. The navigator serves as a liaison between patients, their medical teams and other disciplines to ensure timely access to fertility preservation options and continuity of care. An exciting component of this role will be helping to grow the program through the development and implementation of pediatric fertility preservation services including ovarian tissue and testicular tissue cryopreservation. This position is an excellent opportunity to grow as a healthcare professional and gain expertise in the intersection of medical care and reproductive medicine.

    Responsibilities*

    Care Coordination:

    + Establish rapport with patients and families through proactive outreach at the time of diagnosis or referral.

    + Maintain collaborative communication with referring providers and reproductive endocrinology, urology, gynecology, and other relevant disciplines and team members to ensure coordinated care and minimize delays in starting treatment.

    + Build strong working relationships with program leadership, clinical teams, and administrative partners to optimize workflow and patient experience.

    + Facilitate timely scheduling of fertility preservation consultations, laboratory testing, and procedures.

    Education and Support:

    + Serve as the primary point of contact for fertility preservation questions, providing education and support to patients and families throughout the decision-making process.

    + Assess financial and logistical barriers to fertility preservation and connect patients to available supports (e.g., insurance navigation, grant programs, community resources).

    + Offer psychosocial support and connect patients to appropriate resources, including social work, psychology, child life, or peer support programs.

    + Maintain a current knowledge of institutional and external financial resources for fertility preservation.

    Administrative Responsibilities:

    + Maintain detailed documentation of patient encounters, referrals, and outcomes in the electronic medical record and program databases.

    + Support program growth initiatives, quality improvement projects, and protocol development.

    Required Qualifications*

    + Bachelor's Degree in Nursing, Social Work, Psychology or other related field.

    + Two or more years of progressively responsible experience providing care/case management services.

    Required Knowledge, Skills, and Abilities:

    + Excellent organizational, communication (oral and written), and interpersonal skills.

    + Ability to work independently and collaboratively across multiple disciplines, job roles (e.g., attending and resident/fellow physicians, nurses, social workers, students), and settings (e.g., inpatient, outpatient, virtual care).

    + Ability to maintain accurate, timely documentation and handle patient information with confidentiality.

    + Strong customer service orientation and ability to discuss sensitive and personal health topics with professionalism, empathy, compassion and cultural sensitivity.

    + Ability to manage multiple competing demands effectively and efficiently.

    + Demonstrated adaptability, problem solving and critical thinking skills.

    + Passion for learning and a willingness to acquire in depth knowledge of fertility preservation options, treatments that can impact fertility, and related medical care.

    Desired Qualifications*

    + Master's Degree Preferred in Social Work, Nursing, Public Health, Psychology, or related healthcare field.

     

    Work Schedule

     

    This position is on site, Monday through Friday 8AM - 5:00PM

    Union Affiliation

     

    This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

     

    Background Screening

     

    Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
