    Description:

    The primary purpose of your job position is to plan, organize, and implement social services to PACE Organization

    (PO) participants and families. Responsible for but not limited to assessment, treatment, teaching and counseling

     

    participants, caregiver or other appropriate representatives. Social Worker interventions may include individual

     

    participant contacts, appropriate collateral contact, participant and family education, assessment and counseling,

     

    provision of resources, addressing mental health needed, ongoing case management, advocacy to ensure participant

     

    and caregiver needs are met and addressed and disenrollment procedures.

    Social Worker, Part Time (20 hrs per week)

    Purpose:

    Join us in enriching lives and strengthening communities! As a Social Worker at the PACE Organization (PO), your role is pivotal in orchestrating, delivering, and overseeing social services for our participants and their families. From assessment to counseling, education to advocacy, you'll be at the forefront of ensuring holistic care and support.

    What You'll Do:

    + Conduct comprehensive assessments and develop tailored treatment plans.

    + Provide counseling, education, and support to participants and their families.

    + Collaborate with multidisciplinary teams to address diverse needs effectively.

    + Advocate tirelessly to ensure participant and caregiver needs are met.

    + Navigate disenrollment procedures with empathy and professionalism.

    Minimum Qualifications:

    + Master's degree in social work coupled with two years of relevant experience.

    + Preferably licensed in social work within our state.

    + Experience in geriatric social work is highly valued.

    + Proficiency in social service principles and funding mechanisms.

    + Valid driver's license and reliable transportation are must-haves.

    + Ability to navigate physical demands with ease and grace.
