    The Care Coordination Managerat VNH leads the centralized team responsible for intake, referrals, and scheduling, ensuring patients move smoothly from referral to admission through standardized workflows and reliable data. This role serves as the primary escalation point for coordination issues, partners closely with clinical and program leaders, and drives process improvement using technology, metrics, and clear performance standards. The manager strengthens operational efficiency and enhances both patient and referral source experience by aligning capacity, reducing manual work, and monitoring key KPIs.

    The Care Coordination Manager will:

    * Leads the central Care Coordination Team responsible for intake, referral processing, and scheduling across all programs.

    * Ensures patients move smoothly and quickly from referral to admission through standardized workflows and reliable data.

    * Serves as the primary escalation point for intake and scheduling issues, partnering with program leaders to resolve problems and communicate back to referral sources, patients, and families.

    * Works closely with clinical managers to align referrals with pod capacity, geography, and skill mix.

    * Drive process improvement and change management to streamline operations and elevate the patient and referral source experiences.

    * Use technology and data to track referrals, reduce manual work, and improve visibility.

    * Establish clear performance standards and monitor KPIs for timeliness, accuracy, and conversion.

    Minimum Qualifications:

    * Two (2) years of experiences in a direct supervisory role.

    * Experience in health care setting with working knowledge of medical terminology, electronic medical record and billing systems/software.

    * Bachelor’s degree preferred

    * CHAM - Certified Healthcare Access Manager desired, or certification within 2 years of hire based on organizational needs

    Salary Range (depending on experience):

    * Minimum Pay:$35.00

    * Maximum Pay:$61.75
