*Position Details:* The Hospital of Central Connecticut is seeking a Nurse Practitioner or Physician Assistant to join their Palliative Care team! The Advanced Practitioner (APP) will work in collaboration with the attending physician and members of the interdisciplinary care team to deliver expert palliative care services to patients and their families living with chronic serious or acute life threatening illness, or those nearing end of life. In consultation with the physician(s) and care team members, Physician Assistants/Nurse Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
