    We are seeking a detail orientated and dependable Employment Case Manager to join our Community Employment Services team!

    Key Responsibilities:

    + Connect with community employers to secure paid work and volunteer opportunities for referred individuals.

    + Research and recommend appropriate community employment sites; complete vocational assessments, site evaluations, and follow-up services.

    + Conduct career exploration by identifying vocational opportunities and matching employer needs with client goals.

    + Assist clients with job applications, interview preparation, job skill development, and education on workplace expectations (including ADA information).

    + Teach and reinforce positive behaviors, professionalism, and appropriate social interactions following Clarity Care’s proactive philosophy.

    + Provide on-site job support to both workers and employers.

    + Maintain accurate case management records, including progress notes, contact logs, and required documentation.

    + Serve as a backup Job Coach when needed.

    Ideal Candidate Profile:

    + Bachelor degree in Human Services or a related field preferred.

    + Experience working with persons with varying abilities.

    + Ability to work a flexible schedule according to the needs of the clients and employers.
