    The Registered Nurse Visiting Nurse Advocate works as a designated nurse case manager with county children, youth and family caseworkers to implement the VNAC home visitation program. The VNAC team follows established program policies and appropriate agency policies. Services are provided in the home or other areas designated by county caseworkers.

    JOB DUTIES AND RESPONSIBILITIES:

    + Admits family to VNAC Program and functions as the case manager for the family to assist them in achieving their goals related to health parenting skills and life skills.

    + Completes a family assessment on all referrals from county children, youth and family caseworkers for VNAC services.

    + Establishes family goals and a plan of care with parents and the county caseworker.

    + Works with the family to implement the plan of care.

    + Is responsible for providing health teaching, health monitoring, parenting and life skills education to the targeted individual and family.

    + Provides ongoing monitoring of the success of the plan of care and adapts plan as necessary.

    + Provides ongoing evaluation of plan and measurement of outcomes.

    + Provides ongoing assessment to and collaborates with county caseworkers to determine service frequency, change in plan of care and discharge from service in compliance with VNAC policy and agency philosophy.

    + Communicates with the family’s health care providers orally or in writing as necessitated by the client’s condition and VNAC policy.

    + Maintains and uses clinical records, reports and other written communications according to agency policy and directives. Visit records are written within two days after the visit has been made. New admission records should be completed within 72 hours of admission. Records make clear the goals and plans and document client’s progress and compliance towards meeting the goals. Discharged client’s records will be completed and turned in for processing within 30 days.

    + Participates in county/agency quarterly reviews and other meetings that involve the family.

    + Is responsible for:

    + Writing case summaries.

    + Appearing in court upon subpoena and upon caseworker request.

    + Communicating with legal representatives.

    + Carrying out court recommendations within agency policy.

    + Uses own transportation to provide care at patient’s residence: Must have and maintain valid PA or NJ Driver’s License; Must have and maintain a good driving record accepted by the Network insurance carrier; Must have and maintain motor vehicle insurance with the minimum requirements established by PA or NJ; Must obey and adhere to all motor vehicle regulations and laws while operating vehicle; Must perform routine maintenance on vehicle.

    PHYSICAL AND SENSORY REQUIREMENTS:

    Physical Demands: Heavy Work Exertional activity. Occasionally lift and/or carry 100 lbs., frequently lift and/or carry 50 lbs., and stand and/or walk at least 6 hours in an 8-hour work day.

     

    Non-exertional activity: Stooping (bending at the waist) and crouching (stooping and bending at the knees) – required frequently; handling (grasping) – required frequently; and fingering (fine manipulation) – required only occasionally.

     

    Important: The functional capacity to perform heavy work includes the functional capacity to perform medium, light and sedentary work.

     

    Good physical and mental health. Visual and auditory acuity required to provide comprehensive nursing care.

    Environmental Conditions:

    Inside – Office environment and patient homes.

     

    Outside - Traveling to cases and/or clinics in extreme cold and heat, wet and/or humid conditions, snow, ice and/or fog.

     

    Hazards - Exposure to contagious disease, road and driving, and domestic pets.

    EDUCATION:

    A graduate of an approved school of professional nursing required

     

    Bachelor's Degree in Nursing preferred

     

    Registered Nurse License required

    TRAINING AND EXPERIENCE:

    Two years as a professional nurse in nursing practice. Previous community health experience and/ or Maternal-Child or Pediatric nursing experience is highly recommended.
