Family Nurse Practitioner Enhanced Primary Care

Salary ranges from $120,000.00 to $160,000.00 per year based on experience and education.

Position Summary

The Article 28 Primary Care Clinic is expanding its services and seeking a qualified, compassionate Family Nurse Practitioner (FNP) to join our interdisciplinary healthcare team. The ideal candidate will be committed to improving access to high-quality, person-centered healthcare for individuals with intellectual and developmental disabilities (IDD). This role provides the opportunity to deliver holistic primary care within a clinic designed to meet the unique needs of this specialized population.

Principal Duties and Responsibilities

+ Provide comprehensive primary and preventive care across the lifespan within an Article 28 framework.

+ Conduct assessments, diagnose conditions, and develop evidence-based treatment plans.

+ Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care.

+ Support health promotion, patient education, and chronic disease management tailored to individuals with IDD.

+ Participate in quality improvement initiatives and contribute to the enhancement of clinic workflows and clinical protocols, with a focus on health equity and reducing barriers to care.

+ Provide clinical leadership and mentorship to nursing and support staff, fostering a culture of compassion, compliance, and excellence.

+ Maintain clinical documentation in accordance with regulatory standards and organizational policies.

+ Deliver services both in-person and through telehealth as appropriate.

+ Other duties as assigned.

Qualifications, Skills and Knowledge Requirements:

+ Current New York State Family Nurse Practitioner license and certification (MSN or DNP required).

+ Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine required. Experience with the IDD population is preferred but not required—training and support will be provided.

+ Strong clinical assessment, communication, and interdisciplinary collaboration abilities.

+ Commitment to providing inclusive, equitable, person-centered, and accessible care for individuals with IDD.

+ Willingness to obtain required Intellectual and Developmental Disabilities (IDD)–specific certifications and complete related educational training.

+ Duties require professional verbal and written communication skills.

+ Meet the requirements to drive Springbrook vehicles as required.
