Basic Function: The Family Nurse Practitioner functions as a generalist provider to employees without discrimination to age or gender in a collaborative relationship with the Medical Director of Primary Care using agreed upon protocols. Through expert nursing skill in a broad scope of services, this person is a leader in the clinical management of patients/families in the primary care setting. Beginning with the first encounter, the Family Nurse Practitioner provides identification, management, and or referral of actual and potential problems and strives for the achievement of health maintenance and wellness. Continuity is maintained over time. Requirements: Current New York State license with a certification as a Family Nurse Practitioner. MSN required. Expert nursing practice as RN required.
*Position is also open to qualified Physician Assistant who is licensed and certified in New York State.* Responsibilities: • Functions as an expert clinician in assessing, planning, evaluating, and revising care for the patient across the lifespan of the relationship. • Assumes a leadership role in implementing nursing practices to promote continuous quality improvement and desirable patient outcomes. • Demonstrates leadership in the development, implementation and evaluation of community and patient education programs. • Continually improves clinical avenues to provide optimal care while maximizing resources. • Creates an atmosphere for positive communication across all departments that play a role in the continuum of care. • Adheres to the policies and procedures as outlined in the Exposure Control Manual. • Appropriate Personal Protective Equipment is utilized when performing a task or responsibility associated with this position, which represents a potential occupational risk of exposure to blood-borne pathogens. • Adheres to hospital policies and procedures.

Pay Statment:
Salary Range: $44.23-$80.00/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
