    Responsibilities

    What you will do: Responsibilities listed include but not limited to:

    + Work with insurance and payers to advocate for the appropriate rehabilitation and medical care. Conduct comprehensive assessments of patients' medical, social, and psychological needs

    + Collaborate with the treatment team to develop individualized care plans.

    + Coordinate and monitor the delivery of services, including therapy, medical care, and support services

    + Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey

    + Provide education and support to patients and their families

    Qualifications

    What you will need:

    + Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Nursing, Psychology)

    + Clinical case management experience, preferably in a rehabilitation or healthcare setting

    + Strong interpersonal and communication skills

    + Knowledge of brain injuries and rehabilitation preferred
