Position Overview
Seeking an experienced Director of Case Management to lead hospital case management and social work services within a high-volume acute care environment. This leadership role oversees daily operations, care coordination, discharge planning, regulatory compliance, and performance improvement initiatives.
**Location:** Panama City, FL
**Schedule:** Full-Time | Days (Weekdays Only)
**Hours:** 7–10 hour shifts (occasional on-call, late nights, or extended days)
**Salary Range:** $109,762 – $164,570 (based on experience)
Key Responsibilities
Direct and evaluate case management department operations, staffing, and workflows
Oversee care coordination and discharge planning processes to ensure quality and timeliness
Ensure compliance with hospital policies, regulatory standards, and best practices
Partner with executive and clinical leadership to meet operational and financial goals
Analyze performance data and implement sustainable improvement initiatives
Build strong relationships with physicians, nursing leadership, and ancillary departments
Lead, mentor, and support direct and indirect staff
Qualifications
3+ years of recent acute care hospital case management experience
2+ years of leadership experience
Bachelor’s degree required
Master’s degree in Nursing, Health Administration, or Business Administration preferred
Active licensure required: RN, LCSW, or LMSW

**Pay Details:** $109,762.00 to $164,570.00 per year
