Description
**Job Purpose:** This is a professional position that is responsible for the care and treatment of adults with mental illness. The Case Manager will provide personalized, client-centered care and will work collaboratively with consumers to identify strengths, needs, goals, and a life vision. Under supervision, the case manager will assist in developing and implementing a treatment plan, which will include the necessary follow-up, case management, and intervention as required. This includes connecting clients to behavioral health providers, monitoring medications, accessing AODA services, developing life skills, and linking them to resources.

+ Develop a treatment relationship with a client and formulate treatment plans on his/her caseload that will detail treatment to be provided.
+ Provide client transportation in personal vehicle.
+ Provide supportive community visits in clients’ homes and other community locations.
+ Monitor client’s psychiatric medication regime and compliance as directed by the physician.
+ Provide crisis intervention and support to clients who are in crisis.
+ Responsible for maintaining treatment-oriented environment (under supervision of program supervisor) and assure consistency in carrying out treatment objectives.
+ Coordinate utilization of various community resources, day programs, and outside health care providers.
+ Assists clients in financial planning, development of income resources, and coordinate with representative payee programs as needed
+ Provide treatment related to education and liaison with client’s family and natural supports.
+ Assist client in securing and maintaining adequate housing and prompting and training skills necessary to live independently.
+ Responsible for coordinating the efforts of all professionals and paraprofessionals involved with each client.
+ Responsible for formulation of appropriate discharge plan with assistance of client and program supervisor.
+ Responsible for accurate and timely documentation and maintaining case file as required by agency policy, government regulations and applicable local, state and federal codes.
+ Responsible for meeting department designated monthly billing quotas, EHR note entry within 24 hours and scheduled treatment plans.
+ Attends and participates in staff meetings, in-service trainings, and conferences as required.
+ Providing client coverage for other staff members as assigned.
+ Attendance at community-based MC3 Change Agent meetings as delegated.
+ Required attendance in Trauma Informed Care Trainings
+ Other duties as assigned.
Qualifications
**Required Education, Experience, Certifications, Licensure, and Credentials:** _(Where appropriate,_ _education and/or experience may be substituted)_
**Minimum Required Education:** Bachelor’s degree in the field of Social Work, Counseling, or related Human Services Field
**Minimum Required Experience:** Preferred experience working with adults with severe and persistent mental illness and/or substance use disorders

+ Must be able to assist clients with activities of daily living, such as shopping, cleaning, and cooking.
+ Experience with motivational interviewing is preferred.

Physical Requirements, Visual Acuity, and Work Conditions:
**Physical Requirements:** On occasion they must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Work is performed largely in the community but may at times include an office environment or other various settings.
**Visual Acuity:** The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.
**Working Conditions:** This is a community-based position. Staff are expected to utilize their personal vehicles to transport clients and meet with clients in their homes. This position may work in a variety of indoor and outdoor work environments which may include various outdoor weather conditions, loud noises, fluctuating temperatures, and the occasional use of stairs. The hours of work will generally be during regular business hours and average at least 40 hours a week. This position requires participation in recurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends.
