    • About the Role

     

    We’re looking for a dedicated Clinical Coordinator to join our team and help us continue delivering the level of care our patients and families deserve. In this role, the Clinical Coordinator provides clinical leadership in the oversight of nursing care within the physician practice setting. Responsible for coordinating the clinical and administrative functions that govern the provision of care within the practice. Works closely with the providers and the practice manager to ensure an environment that is caring, healing and welcoming for patients.

     

    • What You’ll Do

    + Policies and Procedures: Responsible for implementing and maintaining nursing policies and procedures for the practice. Participates in quality improvement activities, developing policies, procedures, and/or guidelines to improve patient care and assists with data collection and monitoring.

    + Scheduling: Schedules registered nurses and other clinical staff—including the Clinical Coordinator—to ensure optimal coverage for patient care across both clinic spaces. Organizes and prioritizes work assignments based on patient needs and acuity, applying strong critical thinking skills to maintain efficient workflow. Manages and allocates the distribution of all clinical duties, including telephone triage queues, referrals, clinic staffing, organization and restocking of clinic spaces. Responds promptly to emergency situations in accordance with established protocols.

    + Patient Assessment: During patient visit, assesses patient’s physical/psycho-social status, reviews medications and confirms medication reconciliation as appropriate. Obtains directed obgyn, breast care, and cancer related family history. Obtains vital signs, height and weight. Provides results of assessment to provider and documents findings in the patient’s medical record. Analyzes relevant data and integrates assessment results into care planning, goals, and outcomes.

    + Patient Care: Collaborates with the provider to formulate and implement a plan of care as appropriate based on assessment and diagnosis. Assures consistency in standards of care and the provision of care meets all applicable joint commission standards to ensure patient safety. Triages calls from patients and family members. Engages in relationship-based care to create a caring and healing environment. Advocates for patients and families. Assists in office procedures.

    + Patient Education: Provides information to patient/family with consideration to individual needs. Assesses for barriers to learning and motivation. Consults with other members of the care team to enhance patient and family education. Participates in developing patient education materials. Reviews patient educational materials with patients and their families. Provides patients with appropriate teaching materials resource booklets. Provides preoperative education.

    + Patient Population Management: Communicates imaging and pathology results to patients under the direction of the providers, ensuring clarity and sensitivity. Coordinates follow-up appointments and care as indicated by the providers. Routes orders to providers—including imaging and medications—for review and signature. Triages patient phone calls promptly and collaborates with providers to determine and implement appropriate care plans.

    + Human Resources : Provides ongoing feedback to staff to nurture and develop clinical and interpersonal skills. Proactively addresses issues of conflict. Follows through in a timely manner to staff concerns and questions. Introduces new information to ensure staff skills are progressive and reflect up to date clinical practice. Conducts annual performance evaluations. Ensures staff complete mandatory training as required. Takes corrective action as needed. Consults with Human Resources staff to ensure compliance with human resource policies. Participates in the hiring process of all clinical staff.

    + Communication : Applies patient specific communication skills in interactions that are culturally sensitive and population/age specific. Language barriers and sensory and cognitive deficits are accommodated as needed. Communicates information to the provider that is pertinent, timely and concise. Manages conflict among other team members in a direct and effective manner. Establishes effective communication mechanisms, including but not limited to monthly meetings and one-on-one updates with staff. Participates in departmental and inter-departmental meetings, as needed, and communicates pertinent information to staff and providers.

    + Leadership : Applies leadership skills consistent with role and experience. Delegates and/or assigns care that is consistent with role, scope of practice, and abilities of recipient. Provides feedback pertinent to the situation that is positive and/or provides co-workers educational opportunities. Co-ordinates care within the multi-disciplinary team. Manages conflict in capable, effective manner. Actively participates and promotes professional practice by participating on committees, councils, policy development, unit/hospital projects or professional organizations.

    • What You bring

    + Graduate of an accredited school of nursing, and BSN.

    + Minimum two years clinical experience.

    + Supervisory and outpatient setting experience preferred.

    + Ability to organize and establish day-to-day priorities while utilizing critical thinking skills in all aspects of the job.

    + Effective leadership skills.

    + Ability to provide direct, effective and constructive feedback

    + Must be able to multitask while remaining professional, focused, composed, and positive.

    + Ability to give, take and follow direction in a positive and appropriate manner.

    + Must be flexible to take initiative and embrace new opportunities to grow both personally and organizationally.

    + Must be efficient and effective in the use of resources.

    + Proficient in the use of computers specifically Microsoft Outlook and application of electronic medical record technology as applicable to the practice setting.

    + Current oncology certification in the nursing specialty from an accredited certification program OR completion of 18 hours of cancer-related nursing continuing professional development or continuing education each accreditation cycle (3 years).

    + Expected attendance at Breast Conference (currently every other week).

    + Completion of annual oncology nursing competency assessment
