GENERAL SUMMARY:Under the direction of the Director, Population Health and Ambulatory Clinical Pharmacy Services, the Manager, Medication Therapy Management (MTM) is responsible for the oversight and leadership of the MTM program. The Manager providers leadership for professional staff and supportive personnel. The Manager works closely with leadership to develop and implement strategies which continuously improve the quality of the MTM program. The Manager assists in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization.
EDUCATION/EXPERIENCE REQUIRED:Doctor of Pharmacy degree. Completion of a PGY1 Pharmacy Practice Residency required. Minimum of five (5) years of Medication Therapy Management experience including medication-related performance, progressive leadership experience, and working knowledge of integrated health systems. Minimum of three (3) years of managed care experience preferred. Comprehensive care coordination and management platform (Epic Compass Rose, CareRadius) experience preferred. Must demonstrate the ability to work effectively with others.
CERTIFICATIONS/LICENSURES REQUIRED:Current Michigan licensure as a Registered Pharmacist. Board Certification in Ambulatory Care or other relevant board certification preferred.
