Clinical Pharmacy Services Coordinator

+ Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols
+ Assists with the implementation of pharmacy department initiatives
+ When assigned to distributive pharmacist role, performs all duties expected of that role
+ Supervise pharmacy technician staff involved in the preparation and delivery of medication
+ When assigned to a clinical decentralized role, performs all the duties expected of that role
+ Participates in the provision of pharmaceutical care through clinical rounds
+ Provides education and information on policies, pharmaceutical care and drug therapy
+ Provide patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff
+ Assists in the preparation of materials for the System Pharmacy and Therapeutics committee and other meetings as assigned
+ Complete drug use evaluations (DUE) and new drug and therapeutic class reviews for P&T as assigned
+ Assists in the development of data collection, assessment, and reporting of department quality indicators
+ Serves as pharmacy representative on multidisciplinary committees, as assigned
+ Implements staff development and educational programs
+ Assists with training and onboarding of new pharmacy personnel
+ Assists with the coordination of residents and student learners
+ Maintains competence required for current job title/position
+ Performs other duties as assigned
**Position Requirements** :
+ > 6 years practice experience, Pharm.D. with PGY1 and PGY2 training (or equivalent experience), Illinois licensure, and board certification (specialty certification, as appropriate)
+ Excellent clinical, written/verbal communication, and problem-solving skills
+ Experience supervising pharmacy personnel and precepting pharmacy learners
+ CPR certified
+ Must be creative and innovative in addressing challenges and opportunities.
+ Will be part of overnight rotation when overnight pharmacists are on vacation
