**Pay Range:** $34.89 - $56.78 per hour
Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. The pay range for each role reflects Rush’s anticipated wage or salary reasonably expected to be offered for the position. Offers may vary depending on the circumstances of each case.
Summary:
This position is an integral member of the Rush Community Based Practices (CBP) team and practices in the Rush School Based Health Centers and Adolescent Family Center. This person provides counseling services in the CBP and is responsible for management of other CBP social workers and behavioral health counselors. This person works in a collaborative relationship with other members of the CBP team within Rush and school- and community-based partners external to Rush to support patients, families, and the practices. The person is an expert in biopsychosocial assessment, mental health issues, care planning and coordination of care across medical and community based service systems in order to reduce psychosocial barriers that may negatively impact the patient’s health management and outcomes. This person leads the growth and maintenance of behavioral health programming under the director of the CBP Director, and in collaboration with the CBP and school teams. The individual who holds this position understands population management especially as it pertains to children and adolescents. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush University Medical Center Customer Service Standards.

Required Job Qualifications:
+ Master’s Degree in Social Work
+ Licensed Clinical Social Worker (LCSW) in State of Illinois
+ Computer literate, with high-level organizational and administrative skills.
+ Excellent writing, communication and speaking skills.
+ Initiative, Self-Reflection, Commitment, Responsiveness.
Responsibilities:
• Build and manage innovative programming
• Seek additional revenue streams to cover care management and psychotherapy, to sustain and expand programs.
• Manage and supervise the Clinical Social Workers on the Social Work Services team.
• Manage clinical staff in the provision of the necessary social work services to help clients and families achieve their care goals.
• Maintain relationships with referring Rush clinics through consistent and timely communication.
• Manage the Anne Byron Waud Patient and Family Resource Center and the Tower Resource Center
• Provide mental health counseling or on-demand social work services to own clients or referrals from Rush practices.
• Carry out comprehensive social work assessments by phone or in-person to develop a plan of care for the client.
• Document all client interactions in Epic and maintain HIPPA compliant record for psychotherapy clients.
• Promote Rush University Medical Center clinical services by referring clients to relevant Rush clinics.
Represent Social Work and Community Health at meetings, conferences or other public engagements on an as-needed basis.
