    Availability: Fulltime, M-F

     

    Pay Range: $70,000-95,000

    Responsibilities

    + Conduct comprehensive assessments of patients' health and develop individualized care plans.

    + Coordinate and manage patient care in collaboration with multidisciplinary teams.

    + Monitor patient progress and make necessary adjustments to care plans as required.

    + Educate patients and their families on treatment options and care needs.

    + Provide direct nursing care to patients in their home settings as needed.

    + Document patient condition and care plans accurately in electronic health records.

    + Ensure compliance with healthcare regulations and organizational policies.

    Requirements

    + Current RN licensure in the state of practice.

    + Bachelor's degree in Nursing preferred; relevant experience may substitute.

    + Minimum of 2 years of nursing experience, preferably in home health or hospice settings.

    + Strong communication and interpersonal skills to interact with patients and families empathetically.

    + Ability to work independently and make sound clinical decisions.

    + Familiarity with electronic health records and medical documentation standards.

    + Basic Life Support (BLS) certification required, Advanced Cardiac Life Support (ACLS) preferred.
