    Responsibilities

    + Perform patient evaluations and develop personalized treatment plans.

    + Regularly reassess treatment effectiveness and participate in staffing meetings and family conferences.

    + Conduct discharge evaluations, recommend home and community safety modifications, and provide education and referrals.

    + Communicate patient progress and needs to the treatment team, physicians, patients, and families.

    + Collaborate with other disciplines to coordinate care and enhance patient outcomes.

    Qualifications

    + Degree in Physical Therapy from an accredited program.

    + Active Physical Therapist license in the practicing state.

    + CPR or Basic Life Support certification per state regulations.

    + At least one year of physical therapy experience preferred.

    + Knowledge of rehabilitation techniques for complex neurological injuries.

    + Strong verbal and written communication skills.

    + Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.

    + Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.

    + Health screenings or exams may be required per location or state policies.

    Salary Range

     

    USD $100,000.00 - $112,000.00 / Year
