As the LEAD Program Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:
+ Incorporate the Trans-Theoretical Model of Behavior Change, as applicable into all aspects of supervision
+ Represent CCCCS at forums and meetings pertaining to LEAD
+ Attends and participates in LEAD sub-committee meetings as assigned by agency leadership
+ Work with agency leadership to manage, monitor and meet contract requirements.
+ Develop and maintain Quality Assurance activities including chart review, billing reviews and client satisfaction surveys
+ Create and manage linkages with other service providers in the larger community in order to provide services to those in need
+ Provide orientation to mission and facilitate ongoing discussion regarding mission and its implementation with staff
+ Hire and maintain competent staff.
+ Facilitate Syringe Exchange and safer use supplies access to clients of the program and surrounding counties
LEAD Program Coordinator Salary: $62,353.20
LEAD Program Coordinator Schedule : Monday-Friday 8:30 AM - 4:30 PM (Subject to call)
Qualifications:
+ 2 year/ Associate Degree in Human Services or related field
+ 1-3 years experience
+ Valid driver’s license and access to a reliable vehicle
