Responsibilities
+ Track daily OHS procedures for all surgeons- create a profile for each patient
+ Track all daily readmissions for pts that fall within the 30-day window of post OHS pts
+ Track all daily discharges of OHS pts
+ Call all discharged pts within 24-48 hrs to review discharge medications, enroll into SeamlessMD app and review postop recovery instructions
+ Call all discharged pts one week post discharge to discuss recovery progress
+ Attend daily Safety Huddle Call-Monday-Friday @ 10:30 am
+ Attend and participate in monthly Readmission Reduction and Hospital Performance Improvement Meetings
+ Attend via Zoom meeting with SeamlessMD rep monthly to discuss touchpoints
+ Monitor the SeamlessMD dashboard (Mon-Fri) and respond to inquiries via phone
+ Take clinical calls for post discharge OHS patients for 30-days post discharge (prompted by red rubber bracelet) -eval and treat if necessary in real time
POSTION REQUIREMENTS AND QUALIFICATIONS:
Education:
+ Physician Assistant must be a graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant
+ NP must have a Master’s Degree in Nursing with specialization as a Nurse Practitioner and is certified by the ANCC or AANP organizations within 2 years of employment
+ For PA, Bachelor’s Degree required; Masters preferred
Licensure/Certification:
+ New York State Physician Assistant license required
+ Physician Assistant must have current NCCPA certification
+ New York State Registered Nurse and Nurse Practitioner licenses required
+ Must be BLS/ACLSAED Certified by either the American Heart Association or the American Red Cross. No other organizations will be acceptable
+ DEA license required
Salary Range
USD $76.50 - USD $112.96 /Hr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
