Full Time Case Manager – LPN
New Grads Welcome!
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Pay:
+ Local: $47/hour
+ Mileage Reimbursed: $0.70/mile
Duration: 13 weeks to start (Temp-to-Perm + DP options available)
Position Summary
Amergis Staffing is partnered with a highly respected Adult Day Health organization that provides community-based care services, supporting elderly and disabled individuals to live safely and comfortably at home. Their programs enhance the lives of participants and caregivers through personalized care plans, strong clinical oversight, and ongoing case management.
As an LPN Case Manager, you will assess, plan, implement, evaluate, and revise care while coordinating resources to support program participants. Responsibilities include maintaining patient records, completing assessments, implementing provider orders, and supporting staff development as related to participant care.
Key Responsibilities
+ Complete monthly, in-home visits, providing caregiver education, skills training, and assessing health and environmental changes.
+ Conduct semi-annual visits, reviewing and updating plans of care, goals, medication reviews and reconciliations, and caregiver communication tools.
+ Address concerns related to home environment, psychosocial status, and changes in physical health.
+ Participate in multidisciplinary team meetings and collaborate across disciplines to meet client needs.
+ Complete all required documentation in accordance with regulatory requirements and communicate professionally with client PCPs and other healthcare providers as needed.
+ Demonstrate genuine compassion and cultural humility toward the communities served, fostering trust and inclusivity.
+ Mentor and support team members, contributing to a collaborative and supportive work environment.
+ Perform other duties, responsibilities, and opportunities as assigned.
Qualifications
+ Active Massachusetts LPN license in good standing.
+ 2+ years of nursing experience preferred (new grads welcome).
+ Experience in clinical assessments required.
+ Strong clinical decision-making and care delivery skills.
+ Intermediate to advanced proficiency with Microsoft Office Suite.
+ Willingness to utilize telehealth tools (video, chat, etc.) when appropriate.
+ Experience with electronic medical records.
+ Prior care management experience preferred.
Working Conditions
+ Work performed in residential and community settings.
+ Must travel to member homes; occasional remote work possible.
+ Valid, unrestricted driver’s license; must be mobile across Massachusetts.
+ Member-Facing: YES – in-person client interactions required.
