Job Overview
M Health Fairview is posting a position for a 0.6 FTE Palliative care physician who will be working in the University and community hospitals. The Palliative Care Physician will be a member of the palliative care consultation team and will work in collaboration with other physicians, APPs, palliative care social workers and chaplains to provide timely access and care to patients with palliative care needs. They will provide all necessary acute care services to patients at M Health Fairview, provide timely consultations to requesting clinicians in different settings, ensure proper care, diagnosis and treatment as necessary. The scope of practice of each Palliative Care Physician is defined through the Fairview's Credentialing and Privileging Committees. The Palliative Care Physician is responsible for the care of patients by performing thorough examinations, assessments, and diagnostic evaluations and then development of treatment plan. For this 0.6 FTE position, the palliative care physician's hospital workdays will be proportional to their 0.6 FTE and will be on a rotational basis. Weekend call will also be proportional to the 0.6 FTE.
Responsibilities include:
+ Maintain knowledge of, and comply with, all relevant laws, regulations, policies, procedures, and standards.
+ Actively participate in creating and implementing improvements.
+ Ensure patients receive appropriate care according to their condition, symptoms, and diagnoses. Develop a care plan in conjunction with other physicians and interdisciplinary staff to ensure quality of care. Complete effective and timely communication with coordinating physicians and interdisciplinary staff during the patient's various stages of care.
+ Communicate with patients and families to explain diagnoses, diagnostic or therapeutic treatments, procedures, and prognosis, as well as discuss potential or expected outcomes. Communicate to primary and/or referring physicians. Proactively communicate with other clinicians to effectively coordinate care and timely transfer of information necessary to improve the quality of patient care.
+ Document patient consultation, treatment plan, orders, and clinical progress in the medical record. Dictate, complete records, and submit charges by the deadline as defined by system policy. Returns pages, phone calls, and EMR (Electronic Medical Record) tasks in a timely manner.
+ Work closely with the Medical Director and Clinic Manager to ensure M Health Fairview patient care meets and/or exceeds established requirements. Cooperate and participate in the development of quality improvement initiatives. Attend monthly team meetings, huddles, and other meetings as assigned
Required Qualifications
+ Doctorate (M.D.) or
+ Doctor of Osteopathic Medicine
+ Graduate of an accredited ACGME training in Hospice and Palliative Medicine
+ Board Certified/Board Eligible in Hospice and Palliative medicine
+ Licensed or ability to obtain Minnesota Board of Medical Practice Licensure
+ Ability to obtain and/or maintain DEA certification
+ Meet threshold criteria / qualifications for Credentialing and Privileges

Compensation Disclaimer
The actual rate of pay offered within this range may depend on several factors, such as FTE, skills,knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions withthe same min/max are flat rate jobs based on 40 hours/week. Compensation plans based on production can allow for higher pay than the range posted.
