    SUMMARY

    The Program Director IDD Services Host Home is responsible for supervising the services delivered within the Host Home program and over-seeing the Mentor providers and all individuals served by the program. The Program Director IDD Services Host Home supervises Program Supervisors, Redwood-Host Homes, and Mentor Recruiters. The Program Director IDD Services Host Home is responsible for the operation of the program including case management, Mentor recruitment, advocacy, stakeholder relations, regulatory compliance, staffing, training, financial management, and ensures the quality of services delivered. The Program Director IDD Services Host Home generally works full-time in a supervisory capacity, is considered on-call and expected to be available in the event of emergency. The Program Director works at regional office is generally expected to visit each Host Home regularly.

    ESSENTIAL JOB FUNCTIONS

    To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:

    Services and Supports

    1. Program Planning: Acts as the QIDP (when required), writes and approves individual program plans; trains and assists Program Supervisors with implementation of plans across all shifts.

    2. Records: Periodically audits individual records and other program documentation for accuracy and completion.

    3. Team Meetings: Conducts progress meetings and (quarterly, annual and/or other) reviews as appropriate or determined by the interdisciplinary team; provides and approves written and progress reviews.

    4. Compliance: Oversees licensing compliance for program services and supports; assures timely and accurate program documentation; implements Network compliance plan; manages individual rights implementation.

    5. Management of Individuals' Assets: When applicable, implements system and oversees management of individual’s funds and property, assuring safety and accuracy; audits, monitors and reviews individual’s financial accounts.

    6. Incidents: Implements proactive strategies to reduce or eliminate serious incidents; takes immediate steps to correct potential safety concerns and limit the potential for continued concern or incident; reviews all incidents and directs appropriate response; ensures timely incident reporting; reviews, approves and submits all incident reports as required.

    Stakeholder Relations

    1. People Served: Maintains and fosters communication and relationships with individuals, families, case managers, managed care contractors and other stakeholders; monitors customer satisfaction and implements improvement strategies; advocates on behalf of individuals receiving services.

    2. Public Relations: Communicates where appropriate with the general community, neighbors, and competitors and other outside organizations.

    3. Payer and Government Relations: Maintains and fosters positive relationships with applicable government authorities, such as counties, regional centers and managed care contractors, and all referral agencies.

    4. Political Involvement: Participates in building relationships with local legislators and government officials; makes political contacts and advocates on behalf of the industry, company and persons receiving services.

    Personnel

    1. Recruitment and Hiring: Assists with recruitment and interviewing of all staff; hires Program Supervisors, and Mentor Recuitors in consultation with Area Director; approves hire of Program Services Clinical Coordinators, and other staff members.

    2. Training: Organizes and implements on-site orientation and training for assigned programs; monitors training compliance and documentation; provides orientation, training and guidance to Program Supervisors and Mentor Recruiters.

    3. Employee Scheduling: Designs and monitors schedule.

    4. Evaluation:Conducts annual Program Supervisor and Mentor Recruiter evaluations in consultation with Area Director; reviews and approves annual Program Services Clinical Coordinator and other staff evaluations.

    5. Coaching, Discipline and Termination: Coaches employees, correcting or directing employee behavior where required; may remove employees in the event of emergency; may recommend and participate in decisions regarding discipline and termination in consultation with Area Director and Human Resources; handles termination of Program Services Clinical Coordinators and other staff members as necessary.

    6. Employee Safety: Responsible for employee safety and workers’ compensation programs; monitors compliance; assists in returning employees to work; implements, oversees and trains employees about safety.

    7. Management: Conducts monthly staff meetings; attends monthly regional meetings.

    Growth and Development

    1. Development: Maintains business performance and meets annual growth plans; assists with new start or other core development initiatives where assigned.

    2. Scouting: Promotes relationships with families, case managers and other stakeholders; identifies opportunities by following industry trends and responding to payer need.

    3. Efficiency: Directs program strategy to manage census, maintain occupancy, improve daily attendance and/or maximize utilization.

    Financial Management

    1. Purchasing: When applicable, approves purchase of food, supplies and equipment; reviews and monitors monthly invoices for accuracy and budgetary compliance.

    2. Billing and Utilization: Oversees census, attendance information or other required documentation to assure accurate billing; monitors utilization between authorization and provision of services and supports.

    3. Staffing: Monitors staffing hours for budgetary compliance.

    Mentor Host Homes

    1. May direct the recruitment of Mentors, coordinates pre-service education of Mentor providers, conducts assessments during the assessment and final home certification, helps facilitate the matching process, ensures compliance with the annual recertification of homes

    2. Responsible to ensure all Host Homes meet the Mentor standards as well as those of the funder. Ensures appropriate matching of the individual receiving services to the appropriate independent contractor mentor provider.

    3. Oversees the coordination of program services for the individuals receiving services, their families, and the Mentor Providers. Program services include providing support and guidance to ensure that the intervention, goals and objectives as formalized in the Consumer Program Plans are consistently meeting the preferences and needs of the individual receiving services.

    Other

    1. Performs other related duties and activities as required.

    SUPERVISORY RESPONSIBILITIES

    Directs and supervises the Program Supervisors and Mentor Recruiter, and oversees indirect staff members of the program.

     

    Minimum Knowledge and Skills required by the Job

     

    The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

    _Education and Experience:_

    + Bachelor's degree in Human Service, Master’s preferred

    + Years of related experience per state requirements:

    + **_Ohio_** **:** 1 year of experience

    + **_Pennsylvania_** : 2 years of experience

    + **_All other states_** : 5 years of experience

    + Additional education or experience where required by regulatory authority.

    _Certificates, Licenses, and Registrations:_

    + Current driver's license, car registration and auto insurance if driving on the behalf of the Company or providing transportation to individuals receiving services.

    + QMRP, QIDP, or QDDP certification preferred.

    + Other licensure or certification where required by regulatory authority.

    _Physical Requirements:_

    + **Light work.** Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
