RN Care Manager - Part-Time - Eddy SeniorCare (Latham)
Eddy SeniorCare has a rewarding opportunity for an RN Case Manager to work out of our 385 Watervliet Shaker Road, Latham location!
Eddy SeniorCare PACE is a Program of All-Inclusive Care for the Elderly, dedicated to helping seniors remain in their homes and communities while receiving comprehensive health and social services.
Position Summary:
We are seeking a compassionate and experienced RN Case Manager to join our interdisciplinary team in a Part-Time. This role focuses on providing community-based home visits and coordinating care for PACE participants to ensure optimal health outcomes and quality of life.
**Responsibilities** :
+ Conduct in-home assessments and develop individualized care plans.
+ Collaborate with physicians, social workers, and other team members to manage participant needs.
+ Provide education and support to participants and caregivers
+ Monitor clinical status and adjust care plans as needed.
+ Document all care activities in compliance with regulatory and organizational standards.
Qualifications:
+ Current NYS RN license in good standing.
+ Minimum 1 year of nursing experience (homecare or community health preferred).
+ Strong communication and organizational skills.
+ Ability to work independently and travel within the service area.
Work Schedule:
+ Weekday hours, 8am to 4:30pm
+ 24 hours per week (3 shifts)

Pay Range: $36.50 - $49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
