    Home Health Physical Therapist (PT)

     

    Full-Time | Field-Based

     

    Coverage Area: Lapeer North to Kingston

    What You’ll Do as a Physical Therapist:

    + Deliver high-quality, patient-centered physical therapy in the home setting

    + Evaluate patient needs, develop individualized plans of care, and adjust treatment to support optimal outcomes

    + Collaborate with interdisciplinary team members to ensure coordinated care

    + Educate patients and caregivers to support continued progress at home

    + Provide leadership and guidance to support staff when needed

    + Maintain timely, accurate documentation in accordance with standards of care

    + Serve as a positive ambassador for Elara Caring within the community

    What We’re Looking For:

    + Graduate of an **APTA-accredited Physical Therapy program**

    + Active, unrestricted PT license in the state of practice

    + At least **1 year of experience** in a healthcare setting

    + Comfortable working independently in a field-based role

    + Willing and able to travel up to **50%** within the coverage area

    + Reliable vehicle, valid driver’s license, and current auto insurance

    + Ability to perform physical demands of the role, including lifting up to 50–100 lbs
