At Signature Healthcare Medical Group, the role of Clinical Social Work Supervisor will entail offering high-risk patients all-encompassing psychosocial support. As a vital member of the primary care team, the position assists patients in navigating the medical system and dealing with substance abuse and mental health concerns. In order to evaluate and meet patients' medical, psychosocial, behavioral, and cultural requirements, the clinical social work supervisor works in conjunction with clinicians to provide holistic approach healthcare delivery.
Continuity of Care: Works with interdisciplinary team members to coordinate patient discharge following hospitalization for substance use abuse, mental health issues, and ER visits for high-risk populations.
Patient Assessment and Treatment: Screen, assess, and evaluate patients with mental health and substance use disorders, providing short-term therapy when needed. Utilize evidence-based practices like motivational interviewing and solution-focused therapy.
Subject Matter Expert: Makes ensuring that patient treatment is coordinated by attending departmental, team, and practice meetings.
Mentorship: Offers team members regular clinical supervision as well as direction in challenging situations.
Supervision: Assists with documentation standards and helps Licensed Clinical Social Workers (LCSWs) complete depression screening (PHQ9), thrive screening (social determinants of health, SDOH), and/or other screenings as assigned. Also plans and organizes daily tasks and clinical supervision for LCSWs.
Presentation: periodically and at the request of leadership, presents statistics, workflows, best practices, and modifications to daily duties.
Master of Social Work (MSW) or Master's in Mental Health Counseling from an accredited program required. 3–5 years of experience in a medical setting is required. Licensed Independent Clinical Social Worker (LICSW) required
