Amergis Healthcare is seeking an LPN In Home Case Manager toprovide admissions assessments in Cambridge and the surrounding area. In thisfulfilling role, you will provide community-based careservices designed to help elderly and disabled individuals remain safelyand comfortably in their homes.
LocalPay: $47/hour + mileage reimbursed at $0.70/mile
Qualifications:
+ Active, unrestricted MA nursing license
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Job Duties:
+ Complete intake, annual assessments, and admission visits.
+ Complete all required documentation, including client assessments and multidisciplinary plans of care; coordinate with PCPs and other healthcare providers.
+ Review and educate caregivers on admission binder materials, communication tools, and/or EVV systems.
+ Conduct regular in-home visits, provide caregiver skills training, and assess for health or environmental changes.
+ Conduct semi-annual visits, updating care plans, goals, medication reviews and reconciliations, and caregiver communication tool audits.
+ Identify concerns related to safety, psychosocial status, and physical health.
Duration: 13 weeks with possible extension
