We currently have an exciting full time career opportunity available to join the HKS team as a Direct Support Professional II in our Daytime Habilitation program at our Hempstead location.
The goal of the Direct Support Professional is to empower and direct our individuals in the Day Habilitation program, assist with record keeping, and perform duties necessary for the overall care and well-being of our individuals in this program.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
+ To develop and implement the Staff Action Plan and Data Sheet in accordance with the outcomes detailed in the consumer’s Life Plan/ ISP.
+ Learn the treatment plan methodologies for all individuals in the assigned area, including mobility and all clinical methodologies and implement the methodologies as written.
+ Ensure that other classroom staff are aware of the plans and methodologies.
+ Ensure that active treatment is ongoing.
+ On a daily basis, assure that Medicaid Billing is present, record data accurately and complete the Monthly note
+ On a bi-monthly basis, in accordance with the billing policy, submit required billing materials to the Program Supervisor.
+ Schedule annual and semi-annual treatment team meetings as required by regulation. Notify all participants including outside collaterals of the meeting in a timely fashion (cooking, arts & crafts, trips, etc.) and familiarize other staff with any changes to ongoing individuals’ plan
+ Accompany individuals to community activities (including volunteer experiences) and on trips. Carry out pertinent methodologies with individuals while in the community.
+ Maintain and thoroughly clean equipment and supplies
+ Carry out assignments and/or activities as directed by the professional/supervisory staff.
+ Assist with transporting consumers to/from the Day Habilitation Program, to/from their community placements, and to/from other activities as directed.
+ Act as the liaison to parents, residences, MSC’s, etc. Ensure that they receive pertinent information.
EDUCATION/EXPERIENCE
+ High School Diploma or GED equivalent
+ Experience working with the developmentally disabled
LICENSE/CERTIFICATION:
+ Valid NYS Driver's License (Required)
+ AMAP Certification (Preferred)
+ CPR Certification (Preferred)
+ SCIP-R Certification (Preferred)
SCHEDULE:
+ 8-hour shift
+ Monday to Friday (7:30 am - 3:30 pm)
+ This position requires transporting individuals to and from Brooklyn on HKS Buses. Hours could vary with traffic
PAY RANGE
+ $18.00 to $22.00 per hour
