The physical therapist will accurately assess,

differentially diagnosis, and identify deficits in a patient’s ability to move

and perform functional activities in their daily lives. They develop an individualized treatment plan

with patient-centered goals to reduce pain, restore function, and prevent

disability and work with the individual to prevent the loss of mobility.

Key Responsibilities Include

:

+ Performs patient evaluations through the processof obtaining history, performing relevant systems review, and selecting andadministering specific tests and measures to obtain data.

+ Establishes an individualized plan of care andselects treatment techniques that addresses the specific impairments identifiedin the evaluation and in accordance with physician orders.

+ Conducts ongoing reassessment of the patient andtheir response to treatment.

+ Initiates and manages the discharge planbeginning at the first visit and continuing throughout treatment.

+ Supervises and delegates to ancillary staff andstudents consistent with state laws and department policy.

Required:

+ Graduate of an accredited program of physicaltherapy.

+ Current state Board of Physical Therapylicensure.

+ BLS certification.

Preferred:

+ One year of physical therapist experience.
