Position Summary:
The Maternal Nurse Navigator Registered Nurse is responsible for leading the interprofessional team in continuity of care and planning for the pregnant and postpartum patient. Focused on providing personalized care coordination, education, and support to ensure patients have access to the resources they need for a healthy pregnancy, delivery, and postpartum experience. The nurse navigator collaborates closely in conjunction with community-based organizations, faith-based organizations, worksites and other healthcare providers in the community to promote population health and wellness. The maternal nurse navigator facilitates inte g 1 rated and comprehensive patient services during hospitalization. The maternal nurse navigator provides ongoing assistance and/or consultation to the healthcare team through a comprehensive assessment of patient needs , planning and implementation of an interprofessional care plan, and communication of individual patient need s .
Education:
+ A current licensure as a RN with the Texas State Board of Nurse is required.
+ Certification in Case Management (CCM) preferred
+ Bachelor’s Degree in Nursing required.
Training and Experience:
+
+ + Minimum 3 years of nursing experience in maternal/obstetric care or related healthcare settings.
+ + Current BLS Certification through the American Heart Association upon hire or transfer to department.
+ + Advanced Cardiac Life Support (ACLS) through the American Heart Association within 6 months of hire or transfer to department.
+ + Neonate Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP) within 6 months of hire or transfer to department.
+ + An Introduction to Fetal Heart Monitoring through the Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN) within 3 months of hire or transfer to department. (This requirement can be waived with proof or certification of an intermediate or advanced fetal heart monitoring course taken in the past two years through AWHONN).
+ + Intermediate fetal monitoring course within nine (9) months of hire through the Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN). (This requirement can be waived with proof or certification of the intermediate fetal monitoring course through AWHONN).
+ + Minimum of 1 to 3 years of case management experience preferred
+ + Basic knowledge of health plan products, community resources and alternative funding programs preferred.
