Position Summary
Telework position, can be located anywhere in the US. This position is responsible for member-facing and provider-facing outreach to support Medicare Part D stars initiatives including any current Part D measures. Calls are made to members on the West coast so work hours will be during appropriate call times for the Pacific Time (PT) zone.
Responsibilities of this position include:
• Execute clinical pharmacy programs
• Coordinate strategies for increasing adherence
• Utilize motivational interviewing and health coaching to impact member behavior
•Outreach to members 90% of the day
• Ensure effective collaboration with internal and external matrix partners to support clinical programs
• Work collaboratively with leadership and management to strengthen
business and operational processes
• Other duties as required
Required proficiency in Microsoft Excel, Teams and Sharepoint.
This position could involve travel up to 10%
Required Qualifications
• 1+ years of experience with Medicare Stars, managed care or population health
• 2+ years of customer or patient-facing experience
• Enhanced knowledge of pharmacy and claims processing
• Excellent organizational and analytical skills
• Superior communication skills with patients and clinicians
• Ability to prioritize daily to monthly deliverables
• Computer Skills: Proficient in Microsoft Excel, Microsoft Teams, and Sharepoint
Preferred Qualifications
+ Certified Pharmacy Technician (CPhT), Technician Trainee license, Licensed Practical Nurse (LPN), Certified Medical Assistant (CMA), or equivalent clinical training
+ Knowledge of formulary designs, prescription benefit trends
+ Bilingual in English and Spanish
Education
+ High School Diploma, Associates degree preferred

Pay Range
The typical pay range for this role is:
$46,988.00 - $112,200.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
