    Job Overview

     

    Fairview has an immediate opening for a Cardiovascular Heart Clinic RN at the Heart Clinic located in Maplewood, MN.

     

    This is a 1.0 FTE (80 hours per two week pay period), day shift, benefits eligible. We believe this schedule will be 8 hours, working Monday through Friday between 8:00 a.m. to 5:00 p.m.

     

    Provides nursing care to patients with cardiovascular disease within the scope of practice for a registered nurse. Care is delivered in collaboration with the physicians and healthcare team. Evaluates patients, reviews diagnostic tests and initiates/modifies diet, lifestyle and other therapies using nursing processes.

    Responsibilities

    + Consistent, ongoing monitoring, assessment, documentation, and personalized education of patients with attention to detail and focus on the patient experience.

    + Coordination of care with a variety of disciplines and facilities, to maximize patient outcomes and minimize recurrent hospitalizations.

    + Case Management-style Nursing model with goal-directed care consistent with high quality care.

    + Facilitate Patient Assistance Medication Programs or Grants available to those meeting guidelines Advocate/assist in referral process for appropriate therapies.

    Required Qualifications

    + Associate of Science in Nursing

    + 2 years of post-coronary or coronary critical care experience as a registered nurse.

    + Knowledge of adult cardiovascular health and disease processes as well as related diagnostic and treatment modalities.

    + Ability to perform a brief history and physical examination and to recognize abnormal findings.

    + Basic Life Support (American Heart Assoc or Red Cross)

    + MN Registered Nurse (RN) License

    Preferred Qualifications

    + B.S./B.A.
