    Position Summary

    Provides, arranges for, teaches and/or supervises nursing care through the use of telehealth patient encounters, telephonic follow-up calls, and patient visits. Routine weekend and holiday telehealth monitoring; PTO coverage for the Telehealth Supervisor as needed. Assists with TH program marketing.

     

    Primary Position Responsibilities

     

    + Provides/coordinates patient centered care delivery to patients receiving telehealth home monitoring.

    + Promotes the successful adoption of patient disease self-management behaviors.

    + Program Promotion.

    + Other duties as assigned.

    Qualifications

    Minimum Education :

    Required:

    All hires after July 2012 are required to have a Bachelor’s Degree in Nursing. OR if Associates Degree or Diploma, RN must provide proof of enrollment in a BSN program prior to hire and complete program within five years of hire date.

    Preferred:

    None

    Minimum Years of Experience (Amount, Type and Variation) :

    Required:

    Two years of Med-Surg experience

    Preferred:

    Two years of current home care experience. One to two years of management of high risk patients (cardiac, pulmonary, diabetic)

    License, Registry or Certification :

    Required:

    Current CPR certification, NJ RN licensure, valid driver’s license

    Preferred:

    Certification in specialty

    Knowledge, Skills and/or Abilities :

    Required:

    + Excellent physical assessment skills.

    + Excellent communication skills.

    + Familiar with Microsoft Office & Google Suite.

    Preferred:

    Knowledge of state and federal home health regulations.
