Home Health Physical Therapist

Full-Time | Field-Based

Genesee County, MI

What You’ll Do:

+ Deliver high-quality physical therapy services to patients in their homes

+ Evaluate patient needs, develop individualized plans of care, and monitor progress

+ Collaborate with physicians, caregivers, and interdisciplinary team members

+ Adjust treatment plans to ensure optimal functional outcomes

+ Supervise and guide support staff to help patients achieve therapy goals

+ Maintain accurate, timely documentation and treatment records

+ Serve as a clinical resource and advocate for physical therapy within the community

What We’re Looking For:

+ Graduate of an **APTA-approved Physical Therapy program**

+ Active, unrestricted **Physical Therapist license** in the state of practice

+ Minimum **1 year of experience** in a healthcare setting

+ Comfortable working independently in a field-based role

+ Ability and willingness to travel within the assigned coverage area (approximately 50%)

+ Reliable vehicle, valid driver’s license, and current auto insurance

+ Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
