    Overview

     

    Presbyterian Homes & Services (PHS), through its Optage® Home Health division, is seeking a dynamic and experienced leader to join our team. Optage provides home and community-based services designed to support older adults in maintaining independence, dignity, and quality of life in the place they call home.

    REPORTING STRUCTURE The Home Health Operations Director reports to the Director of Integrated Home Health & Rehabilitative Services. The Home Health Operations Director directly manages the Home Health leadership team, which includes oversight of nursing, therapy, and support personnel.

    Responsibilities

    As a key member of the leadership team, the Home Health Operations Director leads the planning and implementation of strategies that support high quality, integrated services across the continuum of care, drive quality outcomes, and ensure alignment with regulatory requirements and established best practices. Responsibilities include, but are not limited to:

     

    1. Operational Leadership

    • Serve as the Home Health Administrator, accountable to the state for all operations, licensure compliance, survey readiness, and regulatory reporting. • Provide direct oversight of daily operations for Home Health, outpatient, and private duty services. • Ensure effective integration of existing programs (e.g., skilled visits, private duty) with a focus on continuous quality improvement, customer service, health outcomes, staff development, and financial contributions. • Establish and maintain operational policies, procedures, and practices that promote efficient, high-quality service delivery. Ensure all agency policies and clinical protocols are up to date and evidence-based. • Oversee business continuity planning, operational risk mitigation, and survey readiness.

    2. Team Leadership & Collaboration

    • Lead and develop an interdisciplinary team including nursing, therapy, and support personnel. • Create and provide leadership for an effective management team, including talent development, organizational structure, and operating procedures. • Partner closely with Clinical Administrator, Regional Care Managers, rehab leaders, physicians, and other stakeholders to ensure seamless coordination between clinical oversight and operational execution. • Manage complaints or concerns from clients, families, employees, and other site customers in real time.

    3. Performance & Quality Management

    • Oversee incident reporting, patient safety initiatives, and adherence to HIPAA and privacy regulations. • Ensure standards and accountability for capacity management, productivity, scheduling optimization, visit utilization, referral flow, timeliness of care, and documentation compliance. • Monitor clinical capacity and adjust staffing models, caseload assignments, and regional coverage in partnership with regional teams. • Utilize operational dashboards, KRAs, and performance metrics to track and improve agency performance (census, visit utilization, productivity, financial outcomes). • Drive standardization of workflows, processes, and performance expectations across all regions and service lines in alignment with organizational vision, mission, and goals.

    4. Regulatory & Compliance

    • Ensure operational processes support growth targets, regulatory timelines, and Medicare Conditions of Participation. • Collaborate with EMR system administrators and operational leaders to optimize workflows, reduce variation, and improve documentation compliance. • Monitor compliance with state and federal guidelines in all agency operations. • Ensure preparedness for emergencies and disasters affecting patients, staff, and agency operations.

    5. Strategic Planning & Growth

    • Lead strategic initiatives and operational readiness to support innovation, change, and growth. • Provide leadership in the development of annual strategic and financial plans, including operational forecasting and resource allocation. • Achieve or exceed financial, mission, and operational goals by managing revenue, expenses, and capital planning. • Leverage operational data for decision-making using daily management systems, lean problem-solving, and standard work practices. • Act as primary liaison with regulatory agencies, payers, referral sources, and community partners.

    6. Continuum of Care & Specialty Services

    • Ensure smooth operational alignment and continuity of care between short-term rehab, home health, and outpatient therapy services, including back-office functions. • Provide operational guidance for specialty services such as foot care clinics and annual vaccine clinics.

    Qualifications

    MINIMUM QUALIFICATIONS

    • Bachelor’s degree or higher in health care preferred. • A minimum of five (5) years’ experience in health care, preferably in in a Medicare certified home health agency, senior living, and/or outpatient therapy. • Demonstrated ability to write and implement policies and procedures. • Knowledge of State & Federal regulatory requirements for home health and outpatient therapy. Working knowledge of regulatory compliance in the field. • Knowledge of reimbursement under Medicare, Medicare Advantage, commercial payers, and private duty. • Excellent written, interpersonal, and communication skills; adaptability, flexibility, and creativity. • Ability to travel to PHS sites and work a flexible schedule to support site and organization needs. • Excellent interpersonal, communication, and organizational skills. • Proficiency with Microsoft Office, electronic medical records (EMR), and other common computer programs. • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers. • Demonstrated compatibility with PHS’ mission and operating philosophies.

     

    Pay Range

     

    The estimated starting pay range for this role is $103,300-129,000 annually. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.

     

    Benefits

    Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
