    Position Summary:

    Assure residents balanced nutrition needs due to specific medical conditions with food preferences to achieve agreed upon goals.

    Qualifications & Education Required:

    + Bachelor’s Degree in Food and Nutrition or Dietetics required

    + ADA registered or registration eligible required

    + Certified Dietitian/Nutritionist in NYS preferred

    + 1 year in long term care preferred

    + Ability to interact with elders and care for their needs

    + Ability to lead and coordinate team activities

    Position Competencies:

    + Interpersonal skills: support emotional, physical and social well-being of elders and Core Team

    + Problem solving / decision making

    + Technical expertise in Matrix Care, MS Office, and BluePrint

    Position Responsibilities:

    + Responsible for nutritional assessments and monitoring of assigned residents

    + Collaborate with clinical teams to facilitate appropriate nutritional care and care planning on an as needed basis

    + Provide diet counseling to residents and their family members as needed

    + Act as a liaison for policy and procedures pertaining to clinical nutrition

    + Serve as a liaison between foodservice and clinical nutrition needs; including menu updates, product updates and changes and coordination of therapeutic and mechanically altered dietary needs

    + Help with other duties as needed that support the Core Team to fulfill St John’s mission vision and values

    Physical Requirements:

    + Walking, standing for lengthy periods of time.

    Exposure to Conditions:

    + May provide care to individuals with infectious diseases. May be subject or exposed to experiences that involve physical aggression. Occasional use of complex electrical equipment.
