Position Purpose

Join Trinity Health At Home as a **Lead Referral Support Specialist** and play a vital role in ensuring seamless patient care transitions. This position supports our Customer Care Center (C3) by coordinating referral processes, training new team members, and providing administrative assistance to Referral Support Specialists (RSS) and Hospital Care Coordinators (HCC). You’ll be the central liaison for referral management, ensuring accuracy and efficiency from start to finish.

What You Will Do

+ Develop, coordinate, and deliver initial and ongoing RSS training.

+ Oversee RSS weekly and weekend shift assignments.

+ Assist HCCs with referral entry and insurance verification.

+ Accurately document referrals in the EMR system and manage them through completion.

+ Act as the primary contact for referral sources, patients, and families.

+ Ensure timely transmission of referrals for efficient scheduling and meet productivity standards.

+ Respond to RSS inquiries and delegate tasks to maintain workflow efficiency.

Minimum Qualifications

+ High School diploma required; Associate’s degree preferred.

+ 2–4 years of healthcare or related industry experience.

+ Strong knowledge of governmental reimbursement criteria; familiarity with managed care contracts preferred.

+ Proficiency in Microsoft Office; experience with Trinity Health At Home software platforms a plus.

+ Demonstrated computer literacy and ability to navigate multiple systems.
