Phoenix Home Care & Hospice is hiring a Home Health Coordinator to keep our Topeka team running smoothly and our patients supported at every step. If you thrive in a fast-paced environment, love organizing moving pieces, and care deeply about people, this role was built for you.

What You’ll Do
Coordinate schedules for our home health nursing team
Communicate clearly with nurses, clients, and families
Solve scheduling challenges quickly and calmly
Maintain accurate documentation and records
Support office operations as needed
Occasionally assist with weekend on-call coverage
What We’re Looking For
Scheduling or coordination experience (healthcare preferred, not required)
Strong organization, multitasking, and follow-through
Clear, confident communication skills
Comfort with computers and scheduling systems
A compassionate, patient-first mindset
