Full-Time LPN Case Manager

Schedule: Monday–Friday | 9:00 AM–5:00 PM
Assignment: 13 weeks to start (Temp-to-Perm & Direct-Placement options available)
Compensation
+ Pay Rate: $47/hour
+ Mileage Reimbursement: $0.70/mile
Position Overview
Amergis Healthcare Staffing is partnering with a well-regarded Adult Day Health organization committed to helping older adults and individuals with disabilities remain independent and comfortable in their homes. Through individualized care plans, ongoing clinical monitoring, and consistent case management, the team delivers high-quality, community-based support to both clients and caregivers.
As an LPN Case Manager, you will conduct clinical assessments, develop and revise care plans, educate caregivers, and support clients in navigating their health needs. This role is ideal for LPNs seeking meaningful one-on-one work, clinical autonomy, and the ability to make a measurable impact across multiple communities.
Key Responsibilities
+ Conduct intake, admission, and annual assessments in client homes.
+ Complete all required documentation, including assessments, plan-of-care updates, and communication with PCPs and other providers.
+ Educate caregivers on admission materials, communication tools, and EVV systems.
+ Perform regular in-home visits to assess medical status, environmental safety, and caregiver skill proficiency.
+ Conduct semi-annual visits including medication review, reconciliation, care plan updates, and caregiver communication tool audits.
+ Identify and address safety, psychosocial, or health-related concerns.
+ Participate in interdisciplinary Care Team meetings to support coordinated service delivery.
+ Promote a culture of compassion, professionalism, and customer-centered care.
+ Perform additional duties as assigned.
Qualifications
+ Active Massachusetts LPN license in good standing.
+ 2+ years of nursing experience preferred (new grads welcome to apply).
+ Clinical assessment experience required.
+ Strong clinical decision-making, communication, and organizational skills.
+ Intermediate–advanced proficiency with Microsoft Office Suite.
+ Comfort using telehealth tools (video, chat, etc.) as appropriate.
+ Experience with electronic medical records and care management platforms.
+ Valid driver’s license and willingness to travel throughout assigned areas.
Working Conditions
+ Community-based role; work primarily performed in client homes.
+ Occasional remote documentation time possible.
+ Member-facing: Yes – in-person client interactions required.
