Full-Time LPN Case Manager

New Grads Welcome!

Schedule: Monday–Friday | 9:00 AM–5:00 PM

Assignment: 13 weeks to start (Temp-to-Perm & Direct-Placement options available)

Compensation

+ Pay Rate: $47/hour

+ Mileage Reimbursement: $0.70/mile

Position Overview

Amergis Healthcare Staffing is partnering with a respected Adult Day Health organization dedicated to providing high-quality, community-based care for elderly adults and individuals with disabilities. Their mission is to support clients and caregivers in maintaining safety, comfort, and independence through personalized care plans, ongoing clinical oversight, and proactive case management.

As an LPN Case Manager, you will play a key role in evaluating client needs, supporting caregivers, updating care plans, and ensuring continuity of services. This role offers meaningful one-on-one interactions, clinical autonomy, and the ability to make a direct impact within the community.

Key Responsibilities

+ Complete intake, annual, and admission assessments in client homes.

+ Document all required assessments and multidisciplinary plans of care; coordinate with PCPs and other providers as needed.

+ Educate caregivers on admission materials, care communication tools, and EVV systems.

+ Conduct regular in-home visits to monitor health status, safety, and caregiver competencies.

+ Perform semi-annual visits including medication reviews, reconciliation, care plan updates, and communication tool audits.

+ Identify concerns related to medical status, psychosocial needs, home environment, or safety.

+ Participate in interdisciplinary team meetings and collaborate to support client goals.

+ Promote a culture of compassion, professionalism, and customer-focused care.

+ Additional duties as assigned.

Qualifications

+ Active Massachusetts LPN license in good standing.

+ 2+ years of nursing experience preferred (new grads encouraged!).

+ Experience completing clinical assessments required.

+ Strong clinical judgment and communication skills.

+ Proficiency in Microsoft Office; ability to use telehealth tools (video, chat, etc.).

+ Experience with electronic medical records and care management systems.

+ Valid driver’s license and willingness to travel throughout assigned service areas.

Working Conditions

+ Community-based role providing direct, in-home client care.

+ Occasional remote documentation time possible.

+ Member-facing: Yes – in-person interactions required.
