Schedule: Monday–Friday | 9:00 AM–5:00 PM | 24 hours/week
Assignment: 13 weeks to start (Temp-to-Perm & Direct-Placement options available)
Compensation
+ Pay Rate: $47/hour
+ Mileage Reimbursement: $0.70/mile
Position Overview
Amergis Healthcare Staffing is partnering with a well-established Adult Day Health organization dedicated to helping older adults and individuals with disabilities remain comfortable and independent in their homes. Through personalized care plans, ongoing clinical oversight, and consistent case management, this team ensures each client—and their caregiver—receives high-quality, compassionate support.
As an LPN Case Manager, you will play a key role in assessing client needs, developing care plans, ensuring continuity of services, and maintaining clinical documentation. This is a great opportunity for new or experienced LPNs who enjoy autonomy, meaningful client relationships, and community-based nursing.
Key Responsibilities
+ Conduct intake, annual, and admission assessments in client homes.
+ Complete all required documentation, including care plans, assessments, and coordination with PCPs and other providers.
+ Educate caregivers on admission materials, communication tools, and EVV systems.
+ Perform regular in-home visits to assess health status, environmental safety, and caregiver skill competency.
+ Conduct semi-annual visits including medication reviews, reconciliation, and updates to care plans and goals.
+ Identify and escalate concerns related to safety, psychosocial needs, or changes in medical condition.
+ Collaborate with the interdisciplinary Care Team and participate in team meetings.
+ Support a culture of compassion, customer service, and quality care.
+ Perform additional duties as needed.
Qualifications
+ Active Massachusetts LPN license (in good standing).
+ 2+ years of nursing experience preferred (but new grads encouraged to apply!).
+ Experience with clinical assessments required.
+ Strong clinical judgment and communication skills.
+ Proficiency in Microsoft Office; comfort using telehealth tools (video, chat, etc.).
+ Experience with electronic medical records and care management systems.
+ Valid driver’s license and ability to travel throughout the service area.
Working Conditions
+ Community-based role with home visits required.
+ Occasional remote documentation time possible.
+ Member-facing: Yes – in-person interactions required.
