Full-Time RN Case Manager
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Assignment: 13 weeks to start (Temp-to-Perm & Direct-Placement options available)
Compensation
+ Pay Rate: $57/hour
+ Mileage Reimbursement: $0.70/mile
Position Overview
Amergis Healthcare Staffing is partnering with a well-respected Adult Day Health organization dedicated to helping older adults and individuals with disabilities remain safe, independent, and supported in their homes. Through individualized care plans, clinical oversight, caregiver education, and comprehensive case management, this organization provides high-quality, community-focused care.
As an RN Case Manager, you will take a lead role in clinical assessments, ongoing evaluation, and care planning for each client. You will ensure continuity of care, maintain accurate documentation, and collaborate closely with an interdisciplinary Care Team to develop proactive, patient-centered strategies.
Key Responsibilities
+ Conduct intake, annual, and admission assessments in client homes.
+ Complete accurate, timely documentation including assessments, care plans, and coordination with PCPs and other healthcare providers.
+ Educate caregivers on admission materials, care communication tools, and EVV systems.
+ Perform regular in-home visits to evaluate health status, environmental safety, and caregiver competency.
+ Conduct semi-annual visits including medication review, reconciliation, care plan updates, and caregiver communication tool audits.
+ Identify concerns related to safety, psychosocial conditions, or changes in medical status.
+ Participate in interdisciplinary team meetings to support comprehensive and coordinated care delivery.
+ Provide compassionate, culturally sensitive care and promote a strong customer service approach.
+ Perform additional duties as assigned.
Qualifications
+ Active Massachusetts RN license in good standing.
+ 2+ years of nursing experience preferred.
+ Clinical assessment experience required.
+ Strong clinical judgment and ability to work autonomously in the community.
+ Intermediate–advanced proficiency with Microsoft Office Suite.
+ Ability to use telehealth tools (video, chat, etc.) when appropriate.
+ Experience with EMRs and clinical documentation systems.
+ Previous care management experience preferred.
+ Valid driver’s license with the ability to travel throughout assigned areas.
Working Conditions
+ Work performed in residential and community settings.
+ Occasional remote documentation time possible.
+ Member-facing: Yes – in-person client interactions required.
