Physical Therapist (Home Care)

We are currently seeking a Physical Therapist to join our home care team. In this role, you will have the opportunity to make a direct impact by helping patients regain strength, mobility, and confidence in the comfort of their own homes.

As a Physical Therapist at HCR, you will provide skilled, patient-centered care while collaborating with physicians and interdisciplinary team members.

What You’ll Do

+ Evaluate patients using diagnostic muscle, nerve, joint, and functional ability assessments

+ Complete comprehensive initial evaluations and ongoing re-evaluations

+ Develop individualized plans of care, including treatment goals, frequencies, and interventions

+ Recommend modifications to the plan of care to physicians as patient needs change

+ Deliver therapeutic interventions aimed at maximizing functional independence

+ Educate patients and caregivers on treatment plans, goals, and home exercise programs

+ Instruct patients and caregivers on the safe use of prosthetic, orthotic, and durable medical equipment (e.g., canes, walkers, wheelchairs)

+ Document all clinical notes accurately and timely in the medical record

+ Participate in case conferences, staff meetings, and in-service education

+ Contribute to the development and periodic revision of physician Plans of Treatment

+ Ensure adherence to safety standards and proper maintenance of equipment

+ Perform other duties as assigned in support of patient care and team operations

What You Bring

+ Graduation from an accredited Physical Therapy program

+ A current, valid Physical Therapy license issued by the New York State Department of Education

+ At least one year of practical experience as a licensed Physical Therapist

+ Strong clinical judgment, documentation, and communication skills

+ A patient-focused mindset with the ability to work independently in the field

Work Environment

This role involves providing care in patients’ homes and requires travel within an assigned service area. You will work independently while remaining closely connected to a supportive clinical and leadership team.

The working conditions are classified as medium work:

+ Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
