Summary:
The Oncology & Hematology Care Coordinator is a member of the oncology interdisciplinary team that influences and achieves optimal clinical and resource outcomes. The Care Coordinator helps cancer patients and their families navigate the complexities of cancer treatment by serving as a central point of contact, providing patient education, creating care plans, addressing barriers to care and facilitating communication to improve treatment adherence and patient outcomes. The Care Coordinator serves as a change agent for clinical performance and process improvement and provides operational and clinical support to the site Office Manager.
The Care Coordinator is responsible for the delivery of patient care through the process of assessment, planning, implementation and evaluation to the patient's served within the oncology and hematology program. Maintains environmental, infection control and confidentiality standards.
Responsibilities:
EDUCATION
+ Graduate of an accredited school of professional nursing, BSN required
+ Current New York State Licensure
EXPERIENCE
+ One year previous experience in a physician office or ambulatory setting (internal/family med or hematology/oncology) preferred
+ One year previous case, disease or utilization management experience preferred
+ Minimum of 3 (three) years' experience in an oncology setting with telephone triage management skills
KNOWLEDGE, SKILLS AND ABILITIES
+ Successful completion of pharmacy test
+ Ability to evaluate acute/ non-acute situations
+ Demonstrates ability to work effectively in an interdisciplinary team to facilitate efforts in assisting patients with meeting their goals
+ Ability to convey complex clinical information in a simple, concise way
+ Effective verbal and communication skills
+ Ability to teach, motivate and influence others
+ Possesses a problem-solving approach and apply good judgment
WORKING CONDITIONS:
+ Able to work well in stressful situations and conditions of frequent interruption
+ Occasional travel between sites may be required
+ Occasional exposure to unpleasant patient elements
+ Subject to varying and unpredictable situations in a physician office setting
